The pharmacist collaborates with physicians and nurses to optimize pharmacotherapy, participate in rounds, educate patients and staff on safety, monitor outcomes, and ensure compliance with regulatory standards and institutional policies.

Responsabilidades

• Verify and assess medication orders for safety, accuracy, and clinical appropriateness.
• Provide pharmacotherapy recommendations to prescribers and interdisciplinary teams.
• Participate in clinical rounds, consultations, and transitions of care activities.
• Educate patients and staff on medication use, safety standards, and treatment expectations.
• Monitor therapeutic outcomes and adjust recommendations based on evidence and guidelines.
