Qualifications

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
• Licensed in the state or eligible for reciprocity; immunization and non-vaccine injectable certification may be required.
• Residency and/or hospital experience preferred.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on therapy, adherence and safe use.
• Interprets medication orders accurately and participates in emergencies.
• Supports safety/quality improvement and formulary management.
• Prepares, dispenses and ensures proper storage of medications.
• Mentors learners and supervises pharmacy teammates.
