Job summary

Advocate Health’s Pharmacy Department at Advocate Christ Medical Center is seeking a Pharmacist to oversee pharmacotherapy care, provide patient counseling, and ensure safe medication-use processes in a hospital environment.

The role emphasizes evidence-based regimens, formulary management, and collaboration with the clinical team, with rotating AM/PM shifts and a 40-hour workweek. Hospital and pediatric experience preferred.

Qualifications

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
• Licensed in the state(s) of practice or eligible for reciprocity required.
• Immunization and non-vaccine injectable certification required based on area of practice.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provides patient counseling on medication therapy, adverse effects, compliance and proper use.
• Reviews and interprets medication orders and prescriptions accurately; participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities; reports events through appropriate channels.
• Manages formulary adherence and medication-use process; assists in non-formulary and investigational medications.
