Job summary

Trinity Health's inpatient pharmacy in Ann Arbor operates 24/7 to support inpatients, ED, ORs and procedural areas. A Clinical Pharmacist provides medication preparation, distribution, and pharmaceutical care in collaboration with the health care team.

This role requires a Doctor of Pharmacy or equivalent, a Michigan pharmacist license or eligibility, and strong clinical pharmacotherapy knowledge. Hospital residency or prior hospital experience is preferred, with emphasis on CQI and education to

Qualifications

Bachelor's degree in Pharmacy required.
Doctor of Pharmacy degree or equivalent preferred.
Current Michigan Pharmacist license or eligibility required.
BLS/ACLS certification as required by policy.

Responsibilities

Interprets and evaluates medication orders for appropriateness of therapy.
Monitors patient medication regimens for interactions and contraindications.
Individualizes therapy based on patient variables (renal function, age, weight, labs).
Manages drug therapy across continuum of care through communication with physicians and nurses.
Participates in patient rounds, therapeutic drug monitoring, education and antimicrobial stewardship.
Documents clinical interventions and monitors medication therapy outcomes.
Consults with physicians on pharmacotherapy and provides education to staff.
