Job summary

ChristianaCare is seeking a Clinical Pharmacist to join our team at the Newark Campus. The role focuses on direct patient care within an inpatient setting, participating in rounds and collaborating with a multidisciplinary team to optimize pharmacotherapy.

The position requires a PharmD or BS in Pharmacy, Delaware licensure or eligibility, and experience in acute care. Must be willing to work a day/evening shift with a weekend/holiday requirement and participate in precepting students and

Qualifications

• PharmD or BS in Pharmacy from an accredited college.
• ASHP-accredited residency preferred or 3 years acute care.
• Board certification preferred.
• Current licensure (or eligible to reciprocate) in the State of Delaware.

Responsibilities

• Participate in direct patient care through daily interdisciplinary rounds.
• Manage specific medication therapies per approved policies (e.g., warfarin, aminoglycosides, vancomycin, IV to PO conversions, renal dose adjustments).
• Evaluate drug efficacy, monitor therapeutic goals and outcomes, review for contraindications/precautions.
• Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring.
• Identify opportunities during chart review for counseling and medication history completion.
• Communicate recommendations directly with providers.
• Electronically document interventions and provide sign out as appropriate.
• Serve as a preceptor for pharmacy students and residents.
