Overview

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Social Worker - MSW! This position is eligible for up to a $15,000 sign-on bonus for qualified candidates.

Status

Full-Time, permanent position (36 hours OR 40 hours)

Standard working hours

• 3x12 weekend schedule/ 36 hours: 8:00 AM to 8:30 PM EST, Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday, including a 30-minute lunch break
• 4x10 schedule/ 40 hours: 8:00 AM to 6:30 PM EST, Friday through Monday, including a 30-minute lunch break

Eligible for weekend shift differential. The position requires training Monday through Friday from 8:00 AM - 4:30 PM for the first several weeks before transitioning to the full-time weekend day shift.

Job responsibilities

• Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
• Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.
• Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community.
• In consultation with the healthcare team develops effective transition plans.
• Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.

Education

MLD - Master's Level Degree in Social Work / MSW REQUIRED

Certification/Licensure

• Basic Life Support (BLS) - Certification within 90 days of hire.
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience

• 1 year of acute care and/or healthcare experience requirement is defined below:
• Educational internship experience in an acute care facility can be considered OR
• Healthcare experience refers to the care delivered in one of the following settings: long-term care, home health, hospice, Skilled Nursing Facility (SNF)

Compensation & Benefits

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $72,737.60 - $110,323.20 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
