Job summary

Qualifications

• Associate Degree or Diploma RN with a minimum of five years acute care clinical experience or two years case management experience.
• Preference for RN with bachelor or master in nursing and previous utilization management experience.

Responsibilities

• Performs concurrent review of patients admitted to the hospital to determine length of stay, medical necessity and appropriateness of admission.
• Initiates and coordinates discharge planning and resource mobilization with patients/families.
• Assesses psychosocial factors of patients and families and documents treatment plan.
• Acts as liaison among Medical Staff, hospital disciplines, and external agencies.
• Promotes quality patient care and efficient use of resources.
• Assists UR Committee activities and maintains required documentation.
