Lehigh Valley Health Network is hiring an Inpatient Care Manager-Registered Nurse to coordinate comprehensive care for a designated patient population across the continuum of care. The role entails assessing, planning, implementing, and evaluating individualized care plans while ensuring safe and timely discharge planning.

Qualifications

• Bachelor's Degree in Nursing required
• 3 years experience in care coordination, disease management, home health care, hospital, or physician office
• 1 year EMR experience
• Ability to work with diverse backgrounds
• Strong prioritization and coordination skills
• Knowledge of computer applications and analytical tools
• RN license PA/NJ upon hire or equivalent
• Experience with EPIC is a plus

Responsibilities

• Assess and plan care across the continuum of care.
• Coordinate safe and timely discharge planning and post-acute needs.
• Monitor progress and participate in multidisciplinary rounds.
• Educate patients and families about discharge plans and self-management.
• Collaborate with healthcare teams and coordinate conferences as needed.
• Communicate with physicians, patients, and families to resolve issues and coordinate transitions.
