Qualifications

• Bachelor's degree in Nursing.
• 3 years of experience in care coordination, disease management, home health care, hospital, or physician office setting.
• 1 year of EMR experience.
• Ability to interact with persons from diverse backgrounds.
• Ability to set priorities to coordinate care plans efficiently.
• Knowledge of computer applications and analytical tools.
• Proven leadership skills.
• RN - Licensed Registered Nurse; PA license upon hire.

Responsibilities

• Coordinates patient care plans and transitions to achieve optimal outcomes.
• Assesses patients at entry to identify care needs and documents in the EHR.
• Develops, implements, and evaluates multidisciplinary care plans.
• Collaborates with healthcare teams to meet patient needs and improve performance indicators.
• Maintains knowledge of payer benefits and reimbursement methodologies.
