Job summary

Rosaires Care is seeking an RN Care Manager to provide inpatient care coordination and support safe transitions across the care continuum. The role emphasizes comprehensive nursing assessments, individualized care planning, and proactive discharge processes.

You will collaborate with physicians, therapists, social workers, and other clinicians to improve clinical and financial outcomes, educate patients and families, and maintain thorough EHR documentation to support seamless transitions.
