Job summary

Work with nurses, therapists and social workers to coordinate discharge plans and achieve strong financial and quality outcomes. A current BLS certification is required and 3–5 years of inpatient experience in acute, IRF or LTACH

Qualifications

• Licensure in the state where the hospital resides preferred
• Licensed multidisciplinary professionals may include OT, PT, RN, RT, ST or SW
• Current BLS certification required
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH settings

Responsibilities

• Coordinate interdisciplinary care for a designated patient population
• Review inpatient admission records to ensure proper utilization of hospital resources
• Determine admission for the appropriate level of care
• Assess and identify discharge needs
• Coordinate appropriate discharge plans
• Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes
• Maintain and enhance payor relationships
