Qualifications

• 0–1 year post-graduate clinical practice experience.

Responsibilities

• Provides direct clinical services per standards of social/work practice.
• Plans and conducts individual, family and/or group therapy.
• Conducts comprehensive assessments and treatment planning.
• Compiles multidisciplinary treatment plans under physician supervision.
• Performs case management and discharge planning.
• Attends treatment team meetings and integrates reports across disciplines.
• Delivers ethical patient services respecting dignity and cultural needs.
• Coordinates discharge planning with patient support system.
