You will assess risk, support safety planning, and collaborate with psychiatry, nursing, and discharge planning teams to ensure continuity of care and clinically appropriate discharge planning.

Qualifications

• Master’s degree required: MSW or MS in Counseling.
• Licensure: LCSW or LPC or MFT (GA) required upon hire.
• Requires BLS or equivalent certification.

Responsabilités

• Provide individual, group, and family therapy per treatment plan.
• Complete psychosocial and suicide risk assessments; participate in safety planning.
• Collaborate with interdisciplinary team to develop and update treatment plans.
• Document therapy interventions and patient progress in EMR in a timely, accurate manner.
• Assist with discharge planning and coordination of post-discharge resources.
