Job summary

Qualifications

• Must have MSW degree from an accredited program with required field education or internship.
• Two or more years’ experience working with patient populations preferred.
• State licensure such as LMSW/LCSW/LSW is required by the state, CPR and de-escalation/restraint certification is required.

Responsibilities

• Conduct psychosocial assessments and develop treatment plans.
• Coordinate patient cases and discharge planning with families and teams.
• Provide crisis intervention and support to patients and families.
• Lead education, process groups, and referrals as requested.
• Maintain accurate patient documentation and ensure compliance with standards.
• Support recovery and social functioning through therapeutic programs.
