Job summary

MiraVista seeks a Social Worker to join a patient-centered care team in Holyoke, MA. This full-time, day-shift role focuses on direct and indirect patient care, collaboration with physicians, and comprehensive discharge planning within an inpatient setting.

The ideal candidate will hold LCSW/LICSW/LMHC credentials and a Clinical Master’s Degree, with experience in behavioral health and EHR systems. Bilingual English/Spanish is preferred.

Qualifications

• LCSW/LICSW/LMHC required in Massachusetts.
• Clinical Master’s Degree required.
• Experience in inpatient behavioral health or acute care settings preferred.

Responsibilities

• Provide direct and indirect care to patients, families, and guardians as part of the health care team.
• Deliver clinical social work services with physicians and direct care staff.
• Administer biopsychosocial assessments, treatment planning, case management, and therapy services.
• Assist with insurance utilization review and care coordination with hospital and community agencies.
• Contribute to treatment planning and discharge planning with multidisciplinary team.
