Job summary

TaraVista Behavioral Health seeks a Social Worker to join our inpatient team in Massachusetts. You will provide direct and indirect care to patients and families, participate in multidisciplinary treatment planning, and support discharge and aftercare planning.

You must have an LCSW or LMHC licensure eligibility, a clinical Master's degree, and be prepared to obtain CANS certification within 30 days of hire; inpatient behavioral health experience is preferred.

Qualifications

• LCSW or LMHC licensure, or eligibility, required; LICSW/LMHC with inpatient experience preferred.
• Master's degree in Clinical Social Work or related field, required.
• CANS Certification required within 30 days of hire.
• Experience in inpatient behavioral health or acute care settings preferred.
• Advanced knowledge of clinical behavioral health treatment and family systems modalities.
• Proficient typing and computer skills; EHR experience (Medsphere/Vista) preferred.
• Ability to coordinate care and discharge planning.
• Understanding community supports post-discharge.

Responsibilities

• Provide direct and indirect care to patients, families, and guardians as part of the health care team.
• Administer biopsychosocial assessments and contribute to treatment planning.
• Perform case management, individual and family therapy, and systems interventions.
• Assist with discharge planning and aftercare coordination with hospital and community agencies.
• Document goals and interventions to support patient treatment plans.
