Job summary

Endeavor Health is seeking a Behavioral Health Clinical Case Manager I for the Naperville inpatient adolescent program. The role involves psychosocial assessments, treatment planning, and collaborative discharge planning with a multidisciplinary team.

Responsibilities include crisis intervention, documenting care, and coordinating referrals while ensuring alignment with regulatory standards. A social work degree and licensure readiness within the specified window are required.

Qualifications

• Master's Degree in Social work or a Master's Degree in a recognized mental health field.
• 1 year of experience in behavioral health environment preferred.
• If unlicensed, must be eligible to sit for licensing exam within the next 6 months; must obtain LPC/LSW/LMFT licensure within one year of start date.
• Preferred licensures: LSW, LPC, LMFT.

Responsibilities

• Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
• Implement treatment and discharge plans in collaboration with the interdisciplinary team.
• Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.
• Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
• Identify patient and family needs (emotional, social, financial) and escalate complex or high-risk barriers to senior case management staff.
• Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards.
