Job summary

LiveWell Homecare Agency in Danville, PA, is seeking an experienced RN to manage inpatient flow, coordinate transfers, and ensure timely bed placement in collaboration with physicians and interdisciplinary teams.

You will screen admission requests for medical necessity, document transfer plans in the EMR, monitor the admission process to prevent delays, and help identify capacity mismatches while supporting hospital throughput. Night shifts with hybrid options after orientation are offered.

Qualifications

• Graduate from a Specialty Training Program in Nursing (required).
• Minimum of three years RN experience.
• Strong collaboration, communication, and clinical assessment skills.
• Ability to work in a clinical environment with adherence to policies and procedures.

Responsibilities

• Coordinate patient transfers and bed assignments with physicians and nursing teams.
• Assess admission requests for medical necessity and appropriateness.
• Document transfer plans and communications in the electronic medical record (EMR).
• Monitor inpatient bed utilization and address capacity issues to prevent delays.
• Serve as a resource for hospital staff on bed availability and process improvements.
