Qualifications

• Provides substance use disorder assessment and treatment using evidence-based practices.
• Conducts addiction education and therapy for individuals, families and groups.
• Collaborates with multidisciplinary team to plan discharge and treatment.

Responsibilities

• Provide direct patient services for ages 2 and up, including assessments and treatment planning.
• Develop relapse prevention plans with patient and family involvement.
• Maintain accurate documentation of assessments and therapy sessions.
