Pathfinder Home Health is seeking a compassionate Full-Time Licensed Vocational Nurse to cover Conroe and surrounding areas in Texas. You will perform skilled nursing visits, collaborate on patient plans of care, and use the Roger app to streamline charting and stay compliant with OASIS requirements.

Qualifications

• Active LVN license in Texas is required.
• CPR certification is required.
• Prior experience in geriatric home health care preferred.
• Must be a self-starter with strong organizational skills.
• Must demonstrate compassionate care approach.

Responsibilities

• Conduct Skilled Nursing Visits per patient plan of care.
• Collaborate with RN Case Manager to update plan of care.
• Deliver nursing interventions and treatments as outlined in the care plan.
• Observe and document patient status and report changes.
• Educate patients and families on plan of care and self-care.
