Pathfinder Home Health in Texas is seeking a full-time Licensed Vocational Nurse to join a forward-thinking team serving Conroe, The Woodlands, Spring, Tomball, Cypress and surrounding areas.

You’ll use the Roger app, an AI-driven, HIPAA-compliant documentation tool to reduce charting time by 50% while ensuring OASIS compliance, allowing more patient-focused care.

Qualifications

• Active LVN license in Texas is required.
• Current CPR certification is required.
• Prior home health/geriatric care experience is preferred.
• Self-starter with strong organizational skills.
• Compassionate care approach to patients and families.
• Valid driver’s license is required.

Responsibilities

• Conduct Skilled Nursing Visits in accordance with the plan of care.
• Assist RN Case Manager in developing and updating the plan of care.
• Administer nursing interventions and treatments as documented.
• Monitor patient status and report changes to the RN Case Manager/physician.
• Educate patients and families about the plan of care and self-care techniques.
