Job summary

Pathfinder Home Health is seeking a compassionate Full Time Licensed Vocational Nurse to provide skilled nursing visits in the Conroe, The Woodlands, Spring, Tomball, Cypress, and Willis areas in TX. You will work with an RN Case Manager and utilize the Roger app to streamline documentation and care planning.

Active LVN license in Texas and CPR certification are required, along with home health experience. A valid Driver's License and a patient-centered approach are essential for success in this

Active LVN license for Texas
Active CPR certificate
Prior home health experience in geriatrics preferred
Self-starter with strong organizational skills
Compassionate patient-focused approach
Must have valid Driver's License

Responsibilities

Conduct Skilled Nursing Visits per patient care plan.
Assist RN Case Manager in developing and updating plans of care.
Administer nursing interventions and treatments as prescribed.
Monitor patient status and report changes to the RN Case Manager/physician.
Educate patients and families about the care plan and self-care techniques
