Job description

Location

Remote - must live in or near Wilkes County, North Carolina. This position covers caseloads in Wilkes and Ashe counties. Residency in North Carolina or within 40 miles of the NC border is required. This position requires travel.

Job Overview

The Innovations Care Manager (Innovations CM) provides proactive care coordination and intervention for eligible Vaya Health members to ensure appropriate assessment and services. Responsibilities include working with members and care teams to address care gaps, develop care plans, facilitate transitions, and support community integration. This role involves in-home visits and collaboration with community stakeholders.

Essential Job Functions
  • Conduct comprehensive assessments and develop person-centered care plans.
  • Link members with necessary services across health domains.
  • Participate in care team meetings and coordinate services.
  • Support transition and diversion efforts to prevent unnecessary institutionalization.
  • Document care activities accurately and timely in Vaya's systems.
Knowledge, Skills, & Abilities
  • Effective communication and interpersonal skills.
  • Ability to build relationships and work collaboratively.
  • Organizational skills and attention to detail.
  • Proficiency in Microsoft Office and care management platforms.
  • Knowledge of mental health, substance use, I/DD, TBI services, and NC Medicaid programs.
Qualifications

Bachelor's degree in a relevant field and two years of experience working with individuals with I/DD or TBI, or equivalent experience in LTSS. RN licensure or relevant certifications are preferred.

Physical Requirements
  • Visual acuity, mobility, and ability to perform physical activities as needed.
Residency Requirement

Must reside in North Carolina or within 40 miles of the state border.
