Job description

Location

Remote - must live in or near Wilkes County, North Carolina. This position will cover caseloads in Wilkes and Ashe counties. The person in this position is required to maintain residency in North Carolina or within 40 miles of the NC border. This position requires travel.

General Statement of Job

The Innovations Care Manager (Innovations CM) is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Innovations CM collaborates with members and care teams to address care gaps across mental health, substance use, intellectual/developmental disabilities, physical health, pharmacy, LTSS, and other health-related networks. This role involves working in members' communities and supporting transition planning with hospitals and residential facilities. Essential duties include:

  • Utilizing Vaya's Care Management software and health records
  • Engaging in outreach and ensuring HIPAA compliance
  • Performing comprehensive health risk assessments
  • Supporting adherence to medication and care continuity
  • Participating in care planning and interdisciplinary meetings
  • Managing transitional care and diversion from institutional placements
Essential Job Functions

Assessment, Care Planning, and Care Team Collaboration:

  • Conduct assessments and develop person-centered care plans addressing mental health, substance use, medical, and social needs.
  • Link members to appropriate services and supports across health domains.
  • Review clinical assessments, refine treatment goals, and ensure member involvement in care planning.
  • Facilitate care team meetings and monitor progress.

Support, Monitoring, Documentation, and Fiscal Accountability:

  • Participate in multidisciplinary team meetings and community stakeholder collaborations.
  • Monitor service delivery and ensure compliance with standards.
  • Maintain accurate documentation and ensure timely submission of authorization requests.
  • Support member satisfaction and provide educational resources.

Other Duties as assigned.

Knowledge, Skills, & Abilities
  • Effective communication and interpersonal skills
  • Ability to drive and work in rural areas
  • Strong organizational skills and attention to detail
  • Proficiency in Microsoft Office and Vaya systems
  • Knowledge of mental health, substance use, and IDD/TBI services
  • Understanding of NC Medicaid programs and related regulations
Education & Experience

Bachelor's degree in a relevant field, with specific experience depending on the population served, including:

  • Two years working with individuals with I/DD or TBI

Additional requirements include licensure (e.g., RN license for nurses) and meeting North Carolina Qualified Professional criteria.

Physical Requirements
  • Visual acuity, physical activity, and ability to sit and drive for extended periods
Residency

Must reside in North Carolina or within 40 miles of the state border.
