Job summary

The Charles Lea Center is hiring a full-time Innovation Coordinator to support people to live more effective lives by coordinating nontraditional services, technology, and staff supports across residential, employment, and community settings in Tennessee.

Based in the Montgomery County area, the IC will work under the Regional Manager of Innovative Services and collaborate with providers, guardians, and the community to plan and advocate; a bachelor’s or equivalent experience required.

Qualifications

  • A bachelor’s degree from an accredited college or university in a human services field.
  • Or a bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience.
  • Or an Associate degree and 2 years of relevant experience.
  • Or High School Diploma or GED with 4 years of relevant case management experience providing or coordinating services.
  • *Relevant experience means experience working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.

Responsibilities

  • As an Innovation Coordinator, you provide services and supports focused on achieving goals for the people we support.
  • Provide technical assistance to Community Providers implementing services for people referred
  • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
  • Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services and those desired outcomes are being achieved.
  • Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).

Innovation Coordinator - Tennessee

Charles Lea Center is looking to hire a full-time Innovation Coordinator in Montgomery County/Surrounding counties.

Please note, you do not have to reside in Montgomery County but will have to serve Montgomery County and surrounding counties.

An Innovation Coordinator earns a competitive wage of $22.84-$24.04/Hour. Compensation within the posted pay range is determined based on education and experience.

An Innovation Coordinator (IC) works under the guidance of the Regional Manager of Innovative Services. The primary focus of the IC is to support people to live more effective and meaningful lives by working with them to secure nontraditional services that encompass a combination of staff supports and enabling technologies. This includes residential, employment, and community supports. Key areas include assessment, person centered / MAP's planning, linking people to services, training, and advocacy. We are looking for an Innovation Coordinator who is grounded in person centered thinking and one who can foster a creative and forward-thinking approach to providing services in Tennessee.

The Charles Lea Center is a CQL accredited organization that is based in Spartanburg South Carolina and provides services in South Carolina, and North Carolina and recently was awarded a contract with the Tennessee Department Disability and Aging (DDA) to provide services in their State. MINIMUM QUALIFICATIONS
  • At least 18 years of age
  • Must be able to effectively read, write, and communicate verbally in English
  • Ability to pass a background check upon hire

  • This role involves regular standing, walking, lifting (up to 25 lbs), and hands-on support. Reasonable accommodations are available.
REQUIREMENTS:

A bachelor’s degree from an accredited college or university in a human services field;

or

A bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience;

or

Associate degree and 2 years of relevant experience;

or

High School Diploma or General Equivalent with 4 years of relevant case management experience providing or coordinating services to people with disabilities and special needs and/or with case management services.
