UNC Health in Apex, NC seeks a skilled RN to assess and document patient status and formulate education plans for infusion patients in a skilled nursing facility. This per diem role requires a NC RN licensure and a BSN is preferred.

Responsibilities include maintaining patient safety, developing and implementing individualized care plans, collaborating with the health care team, and ensuring accurate documentation. Night shift work with on-site assignments.

Qualifications

• Must hold NC RN licensure.
• BSN from an accredited school preferred.
• At least 1 year of experience in skilled nursing facility.

Responsabilités

• Maintain a safe environment for patients, staff and self.
• Assess patient condition including biophysical, psychosocial and age-specific needs.
• Document patient status per unit procedures.
• Develop an individualized care plan using nursing and medical diagnoses; collaborate with the care team.
• Implement the nursing care plan for assigned patients.
• Evaluate patient status and outcomes of care; adjust plan as needed.
• Initiate a patient education plan per physician/policy; include patient and family instructions and discharge planning.
• Maintain required education as required by the nursing board.
