Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops a relationship with patient and multidisciplinary team to facilitate treatment and follow-up.
• Completes a nursing assessment to identify the patient's individualized needs and collaborates with the team to develop an interdisciplinary plan of care.
• Supports patient and family by providing education, addressing psychosocial needs, advocating, and ensuring continuity of care.
• Navigates patients and family through diagnosis, treatment, and follow-up of the continuum of care.
• Coordinates teaching plans and discharge planning with the multidisciplinary team to support wellness and health maintenance.
