• Rph Ohio - Registered pharmacist license required.

Responsibilities

• Provide pharmaceutical care, primarily for intravenous therapy in home care patients.
• Consult with physicians, nurses, third party payers, caregivers and patients to plan, implement and monitor therapy.
• Assess drug orders for accuracy and appropriateness; develop care plans and maintain medication profiles.
• Monitor patient progress and outcomes; compound sterile products and select appropriate supplies.
