Job summary

Amerita, a leader in home infusion therapy, is seeking a Pharmacist to join our Hudson, MA pharmacy team. You will perform pharmaceutical care activities for customers and patients, reporting to the Pharmacy Manager.

The role emphasizes collaboration with physicians, nurses, and distribution, with ongoing professional development discussions. Responsibilities include reviewing orders, ensuring safe drug therapy, professional documentation, and on-going patient assessments.

Qualifications

• Bachelor of Science in Pharmacy is required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two years of acute-care experience or one year in home infusion.
• Experience with compounding infused drugs.
• Excellent verbal and written communication skills.
• Flexible schedule with on-call evenings and weekends.

Responsibilities

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians and nurses.
• Adhere to all applicable laws and standards governing pharmacy practice.
• Coordinate referrals and admissions with nurses, customer service and distribution.
• Review and verify new patient orders and document accordingly.
• Assist physicians and other professionals with home care patient planning and monitoring.
• Document and monitor drug therapy outcomes and lab data.
• Provide drug information and consultations to patients and healthcare professionals.
