Job summary

Amerita, Inc. is seeking a Pharmacist to join our home infusion Pharmacy team in Prescott, AZ. The Pharmacist will report to the Pharmacy Manager and support patient care through evaluation of drug therapy and collaboration with physicians, nurses, and distribution staff.

The role requires at least a BS in Pharmacy with licensure in the state and 2+ years acute-care or 1 year home infusion experience. On‑site work with potential travel, and opportunities for professional development are included.

Qualifications

  • BS in Pharmacy required; PharmD preferred.
  • Licensed Pharmacist in state(s) of practice.
  • 2+ years acute-care OR 1 year home infusion experience.
  • Experience with compounding infused drugs.
  • Strong organization and time‑management skills.
  • Excellent verbal and written communication.

Responsibilities

  • Evaluates and monitors drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and documents on the respective order form.
  • Assists the physician and other healthcare professionals with patient care planning, setting therapeutic objectives and monitoring safety and efficacy.
  • Monitors outcomes of drug therapy by evaluating laboratory and clinical data; documents activities and communicates with patients and professionals.

Responsibilities
  • Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditation and participates in quality assurance activities.
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and documents on the respective order form.
  • Assists the physician and other involved healthcare professionals with:
    • Planning and implementing patient care in the home;
    • Setting therapeutic objectives for drug therapy;
    • Establishing parameters for monitoring the safety and efficacy of drug therapy;
    • Preparing regular progress reports.
  • Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in‑services, seminars and conferences.
  • Compounds sterile mixtures and non‑mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
  • Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.
  • Documents all activities on appropriate logs.
  • Pharmacists’ duties are extended to include overseeing Pharmacy technician functions as required by Pharmacy Board rules.
Qualifications
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two (2) years of recent experience in an acute‑care setting or one (1) year of recent experience in home infusion therapy.
  • Experience with compounding infused drugs.
  • Proven record of evaluating and monitoring drug therapies for home care patients.
  • Good organizational and time‑management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Excellent verbal and written communication skills.
  • Flexible schedule with the ability to work on‑call evenings and weekends on an as‑scheduled basis.
  • Percentage of Travel: 0‑25% – minimal travel may be required.
Physical Requirements

To perform this role will require constant sitting and typing on a keyboard with fingers, and occasionally standing, and walking. The physical requirements will be the ability to push/pull and lift/carry 10‑30 lbs.
