Amerita, Inc. is seeking a Pharmacist to join our Pharmacy team in the Irving, TX area. You will evaluate and monitor drug therapy for home care patients, collaborate with physicians and nurses, and ensure adherence to policies and regulations.

The role requires a Pharmacy license, ASHP-style documentation, and the ability to work flexible evenings/weekends. Experience in home infusion and compounding is preferred.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two years of recent experience in an acute-care setting or one year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Good organizational and time-management skills.
• Participation and membership in professional societies and organizations is desirable.
• Excellent verbal and written communication skills.
• Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis.
• Travel: 0-25% may be required.

Responsibilities

• Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
• Adheres to all appropriate standards of accreditation and participate in quality assurance activities.
• Coordinates and facilitate new referrals and admissions through collaboration with nurses, customer service and distribution.
• Reviews, receives and verifies all new patient orders and documents on the respective order form.
• Assists the physician and other involved healthcare professionals with planning and implementing patient care in the home, setting therapeutic objectives, establishing monitoring parameters, and preparing progress reports.
• Conducts and documents initial and ongoing assessment of patient, drug and disease information to identify drug-related problems.
• Monitors outcomes of drug therapy by evaluating lab/clinical data and patient information.
• Provides consultation and answers inquiries from patients, families, physicians and healthcare professionals regarding drugs and their use.
• Maintains thorough documentation in patient records of clinical activities and communications.
• Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
• Maintains clinical skills and knowledge by reviewing literature and attending in-services.
• Compounds sterile mixtures and non-mixtures per state laws (parenteral nutrition, antibiotics, chemotherapy, hydration, narcotics).
• Ensures compounding/clean room areas and equipment meet policies and USP 797/800 standards.
• Documents activities on logs and maintains logs accordingly.
• Oversees Pharmacy technician functions as required by Pharmacy Board rules.

• Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
• Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
• Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
• Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in‑services, seminars and conferences.
• Compounds sterile mixtures and non‑mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
• Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.
• Documents all activities on appropriate logs.
• Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in‑services, seminars and conferences.
• Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules.

Physical Requirements

To perform this role will required constant sitting and typing on a keyboard with fingers, and occasionally standing, and walking. The physical requirements will be the ability to push/pull and lift/carry 10‑30 lbs.
