Job summary

Trinity Health Ann Arbor seeks a Clinical Pharmacist for the Infusion Pharmacy to ensure safe preparation of infusions and optimize patient care. You will monitor therapies, review orders, and collaborate with care teams on high-risk treatments.

Qualifications

• Bachelor's degree in Pharmacy is required; Doctor of Pharmacy preferred.
• Hospital experience or residency strongly preferred.
• Licensed Michigan pharmacist; license for Controlled Substances required; BLS/ACLS as required.
• Must communicate with health team and patients.
• Thorough knowledge of pharmacokinetics, therapeutics and pathophysiology.

Responsibilities

• Interprets medication orders for appropriateness of therapy, dosage, route, and duration.
• Monitors patient regimens for interactions, allergies and adverse effects.
• Individualizes therapy based on patient variables such as renal function and lab values.
• Collaborates with physicians and nurses to optimize drug therapy and resolve unclear orders.
• Participates in rounds, drug monitoring, teaching and counseling.
