Qualifications

• Associate degree required; LPN or RN licensure.
• Infusion certification required for LPNs.
• CPR certification required.

Responsibilities

• Monitor and document patient conditions and reactions.
• Administer medications and IV fluids; record details.
• Record vital signs: height, weight, BP, respiration.
• Collect samples for testing as needed.
• Educate patients on health maintenance and community resources.
• Coordinate smooth transitions of care across settings.
• Use health information systems to manage and improve care.
• Incorporate quality indicators into care plans.
• Discharge patients after therapy completion.
• Participate in team meetings and trainings on best practices.
