Job summary

IVX Health in Fremont, CA is seeking a Float Nurse Practitioner to deliver outpatient infusion care across Fremont, San Jose, San Mateo, and Sunnyvale. This full-time role covers Monday–Friday with every other Saturday shifts, 7:00 AM–7:00 PM, and may vary by location.

You will oversee infusions and oversee patient care with a multidisciplinary team. You will supervise care, perform assessments, educate patients and families, and ensure HIPAA-compliant documentation.

Qualifications

  • Active and unrestricted NP and RN licenses.
  • Board certification required (AANP or ANCC).
  • BLS required; PALS preferred.
  • Experience in ambulatory infusion centers or outpatient clinics is a plus.
  • Ability to treat both adult and pediatric patients.
  • Strong IV/Infusion therapy skills.
  • Proficient in clinical documentation and HIPAA/compliance.

Responsibilities

  • Provide direct patient care with on-site and virtual supervision for infusions and injections.
  • Conduct patient assessments and document SOAP notes.
  • Educate and support patients and families on treatment expectations and side effects.
  • Maintain accurate EHR records and uphold HIPAA/OSHA standards.
  • Assist with chart preparation and treatment planning to streamline visits.
  • Collaborate with referring providers and cross-functional teams.
  • Assist in inventory management and prior authorizations.
  • Work at multiple facilities within the market as needed.
  • Perform additional duties as assigned.

What You Will Do

  • Provide Direct Patient Care Deliver on-site and virtual supervision while administering infusions and injections with precision.
  • Conduct Patient Assessments Perform evaluations, document SOAP notes, and monitor treatment responses.
  • Educate and Support Patients Guide patients and families through treatment expectations, potential side effects, and disease management.
  • Ensure Compliance and Quality Maintain accurate records in the Electronic Health Record (EHR) system, review nursing documentation, and uphold HIPAA and OSHA standards.
  • Optimize Clinical Workflows Assist with chart preparation and treatment planning to streamline patient visits.
  • Collaborate with Healthcare Teams Work closely with referring providers and cross-functional teams to enhance care coordination.
  • Support Operations and Training Assist in inventory management, prior authorizations, and regulatory adherence.
  • Flexibility and Adaptability Work at multiple facilities within a designated market as needed.
  • Perform Additional Duties Take on other responsibilities as assigned to support patient care and organizational objectives.

What We're Looking For

  • Active and Unrestricted Licenses Nurse Practitioner (NP) license and Registered Nurse (RN) license (if required by the state).
  • Board Certification AANP or ANCC certification required.
  • Life Support Certifications BLS certification required; PALS certification preferred.
  • Clinical Experience Background in an ambulatory infusion center, outpatient specialty clinic, or similar setting is a plus.
  • Ability to Treat a Diverse Patient Population Must be able to care for both adults and pediatrics Family Nurse Practitioner (FNP) preferred.
  • Strong IV and Infusion Therapy Skills Experience with specialty biologics, medication management, and emergency response protocols.
  • Proficiency in Clinical Documentation Experience with EHR systems and a strong understanding of HIPAA and regulatory compliance.
  • Exceptional Communication & Collaboration Ability to work effectively with patients, families, and multidisciplinary teams.

Pay is based on factors such as market location, job-related knowledge, skills, and experience, and is benchmarked against similar organizations in our size and industry. It is not typical for an individual to be hired at or near the top of the posted range, as compensation decisions depend on the facts and circumstances of each case. In addition to cash pay, full-time regular employees are eligible for 401(k), health benefits, and other company-provided benefits.
