UPMC Chartwell is seeking an Infusion Nurse to coordinate and perform skilled nursing care in patients' homes in the Pittsburgh area. The role emphasizes patient education, leadership within the care team, and adherence to protocols with weekend shifts and a 9:00 am–5:30 pm schedule.

The position requires nursing licensure with CPR certification; BSN is preferred and prior home health and infusion experience are highly valued.

Qualifikationen

• Diploma from an accredited nursing school required.
• B.S.N. preferred.
• 1 year of home health experience preferred.
• 1 year of infusion therapy experience including venous access.
• CPR certification required; PICC training preferred.

Aufgaben

• Participate in the Chartwell Pennsylvania Performance Improvement Program.
• Provide infusion and other skilled nursing care in patients' homes.
• Take on-call responsibilities as assigned to provide 24-hour availability.
• Attend patient care conferences, in-services, and departmental meetings.
• Submit required patient care documentation and reports in a timely manner.
• Maintain patient confidentiality and professional appearance and conduct.
• Communicate patient condition changes to supervisor, physicians, and other health care providers.
• Educate patients and families on home medical management and care plans.
