Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Boulder and surrounding locations. You will educate patients and families on prevention and healthy habits, and provide high-quality nursing care with minimal supervision.

The ideal candidate is compassionate, cool-tempered, and capable of handling emergent situations, following health and safety guidelines at all times.

Qualifications

• Background check required.
• Malpractice insurance required.
• Drug screen required.
• Associate or Bachelor Degree in Nursing; minimum five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER, acute experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS required.
• ACLS; PALS preferred.
• Ports access: implanted ports; central lines; PICC.
• Strong IV placement skills.
• Strong organizational and communication skills.
• COVID-19 Vaccinated preferred.

Responsibilities

• Complete daily EMR forms for each patient.
• Provide admission, case management, and follow-up visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on orders and initial assessment.
• Provide hands-on care, management, and teaching of the patient under a physician's direction.
• Report patient care, conditions, and progress to the physician, pharmacy, and Clinical Manager.
• Implement patient care plan with patient and family to help achieve optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, with necessary documentation.
• Prepare medical documentation on all patients, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update patient records, and confer with other healthcare disciplines to provide optimal care.
