Job summary

Northwell Health seeks a registered nurse to provide age-appropriate care across the patient population. You will use the nursing process to assess, diagnose, plan, implement, and evaluate care, collaborating with physicians and the health care team to coordinate procedures and treatments.

You will lead care activities, ensure patient safety, reassess plans, and educate patients and families. Responsibilities include discharge planning and interprofessional communication to optimize outcomes.

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Nurse in New York State and/or Connecticut.
• BLS certification required; additional certifications as needed.

Responsibilities

• Assess patient’s physiologic health status and review chart; communicate plan of care.
• Assess patient’s psychosocial health status and support needs.
• Identify patient outcomes and establish individualized goals.
• Formulate and prioritize the plan of care; coordinate supplies, equipment and medications.
• Implement the plan of care and monitor safety; educate patient and family.
• Reassess and revise plan as needed; document treatments and patient responses.
• Collaborate with physicians and care team; delegate when appropriate.
• Perform grade I–IV decubitus care and skin care maintenance.
• Administer medications accurately and safely; document administration.
• Provide patient and family education; assist with discharge planning and referrals.
• Perform related duties as required.
