Job summary

Descanso Home Health Services in Cypress, CA, is seeking a compassionate Home Health Registered Nurse (RN) with infusion expertise to deliver high-quality, patient-centered care directly in patients' homes. Responsibilities include administering IV therapies, assessing patients, educating families, and coordinating with physicians.

Must have CA RN license, 2+ years infusion experience, CPR, and a valid CA driver's license.

Qualifications

• Active California RN license in good standing.
• Minimum 2+ years of infusion therapy or IV experience.
• Oasis Charting experience.
• CPR certification.
• Valid California driver license and reliable transportation.
• Strong clinical assessment and patient communication skills.

Responsibilities

• Administer IV infusion therapies including antibiotics, chemotherapy, TPN, and other prescribed infusion treatments in the home setting.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor and document patient responses to infusion therapy and report changes to the physician.
• Educate patients and caregivers on medication management, infusion care, and safety protocols.
• Maintain and troubleshoot infusion pumps, PICC lines, central venous catheters, and other vascular access devices.
• Coordinate care with physicians, pharmacists, and interdisciplinary team members.
• Ensure accurate and timely clinical documentation in compliance with home health regulations.
