Qualifications

Responsibilities

• Assess, plan, implement, coordinate, educate and evaluate age-appropriate care.
• Document data in a retrievable, understandable format and update plans.
• Develop and update individualized care plans for health promotion and problem resolution.
• Prioritize interventions and coordinate with the care team promptly.
• Educate patients and families about care delivery and available resources.
