Job summary

Lucent Health is seeking an Infusion RN to join our Clinical Operations team, supporting the home infusion site of care program. You will review eligibility for home infusion, engage patients by phone, and coordinate care transitions with in-network providers to ensure safe, cost-effective infusion experiences for self-insured employer clients.

You will collaborate across care management, educate patients, and monitor therapy after transition, addressing barriers and ensuring timely

Qualifications

• Registered Nurse with a minimum of 5 years Clinical Experience.
• Experience in infusion therapy, home infusion, or IV therapy strongly preferred.
• Knowledge of home infusion site-of-care transitions and payer/benefit considerations for infusion services.
• Credentials such as CRNI (Certified Registered Nurse Infusion), CCM, CRRN, OCN, or other pertinent certifications (preferred)
• Active, unrestricted multi-state RN license
• Excellent written, telephone, and computer skills, with strong time management and organizational skills
• Positive, proactive, team-oriented approach, with the ability to work independently in a telephonic care management environment

Responsibilities

• Assess clinical eligibility for home infusion based on diagnosis and treatment plan.
• Call patients to discuss benefits of home infusion versus facility care.
• Educate patients and caregivers on the home infusion process and providers.
• Arrange initial home infusion appointment with in-network provider.
• Coordinate transition of care to in-home setting.
• Verify clinical documentation and payer authorizations before transition.
• Communicate with in-network provider on readiness and scheduling.
• Monitor patients after transition for tolerance and adverse events.
• Identify and address barriers to home infusion with care team.
• Develop individualized care plans and set measurable goals.
• Advocate for patients and optimize benefit dollars.
• Document all eligibility reviews and transition activities.
