Expires 07/31/2029
Why are you being asked to complete this form?
We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. To do this, we must ask applicants and employees for the following information: what, if any, disability or functional limitation you have that may affect your ability to comply with the job requirements. Disclosures are confidential and will only be used for ensuring compliance with the law and for determining reasonable accommodations if required. Disclosure of a disability does not guarantee a particular decision about your application or any other employment action.
We will also ask you if you have a disability that requires accommodation while you attend or start work. If you do, we will work with you to determine the accommodation needed. The requirement to disclose a disability or a functional limitation remains voluntary, but we encourage you to share when you can.
PUBLIC BURDEN STATEMENT: According to the Paperwork Reduction Act of 1995 no persons are required to respond to a collection of information unless such collection displays a valid OMB control number. This survey should take about 5 minutes to complete.
