Self-Help for the Elderly in San Francisco seeks an Information and Assistance Specialist to help seniors and disabled adults navigate home- and community-based long-term care, provide assessment, information, referral and follow-up.
Responsibilities include conducting basic needs assessments, assisting consumers in accessing services, documenting services, and developing outreach with the Social Services team. This is a regular full-time position based in San Francisco.

Summary: Assists in the navigation of the home and community-based long-term care system; provides assessment, information, referral and assistance to older adults, adults with disabilities, and caregivers; performs follow-up to ensure delivery of services; collects, updates and shares information on community resources.
