Job summary
The Clinical Specialist Pharmacist, Infectious Diseases and Outpatient Parenteral Antimicrobial Therapy, provides expert-level clinical pharmacy services to patients receiving antimicrobial therapy across the continuum of care.
The pharmacist is responsible for developing, implementing, and supporting a standardized OPAT service and providing comprehensive medication management within ambulatory Infectious Diseases specialty practices.
Qualifications
- Experience providing infectious diseases clinical pharmacy services.
- Experience in outpatient antimicrobial therapy (OPAT) programs.
- Collaborates with interdisciplinary teams to optimize patient care.
Responsibilities
- Provide expert-level clinical pharmacy services across inpatient and outpatient settings.
- Develop and support standardized OPAT services.
- Monitor antimicrobial therapy and stewardship activities.
- Educate patients, caregivers, and staff.
- Collaborate with physicians, nurses, and microbiology staff.
- Lead and participate in committees and protocol development.
