Location/Schedule: NYC - 3 locations 60% remote telehealth from home and 2 days per week in clinic

• Scheduled at any of the 3 sites but not required if provider has site preference
• Client is flexible
• Either in clinic 2 days per week, remote from home 3 days per week or in clinic 1 day per week and remote from home 2 days per week
• Brooklyn, NY
• Lower East Side-New York, NY

• Mon-Fri (9am-5pm)
• 3 month locum w/ option to extend

• Outpatient FQHC Community Clinic
• Typical cases: HIV, Hep C, Hep B
• Position is 60% Telehealth and 40% in office patient contact
• Average 16 patients/day
• Adults only
• EMR: eClinical Works
• Support Staff: MD s, RN s, LPN s, MA s, Medical Director, Office Staff, Office Manager
• Credentialing: 30 Days
• Require: Board Certified in Infectious Disease, NY license, DEA, BLS
