You will support disease prevention, lead a NP team, and collaborate with facility leadership to deliver high-quality care.

Qualifications

• Licensed Nurse Practitioner with SNF experience.
• Minimum 1 year of supervisory or lead responsibilities.
• Experience in primary, acute, and chronic care settings.
• Knowledge of health care regulations and safe practice.
• Excellent communication and strong ethical standards.
• Team player with a positive attitude and willingness to mentor NPs.

Responsibilities

• Examine patients and review medical records.
• Order and interpret diagnostic tests.
• Diagnose health conditions and propose treatments.
• Prescribe medications and manage care plans.
• Maintain accurate records and scheduling.
• Educate patients and families on diseases and prevention.
• Oversee NP team and lead meetings.
• Assist with onboarding and training of new NPs.
