You will work under supervision to collaborate with clinical providers, collect and verify data, and contribute to public health efforts.

The role is grant-funded for a defined period, with duties including outreach, education, field visits, and coordination of care for patients.

Qualificações

• Four years of experience in health services, community outreach, or related intake work.
• Two years with associate degree, or a bachelor’s degree or higher.
• Proof of education, training or experience required.

Responsabilidades

• Obtain and enter laboratory reports from providers and facilities.
• Contact patients for follow-up and treatment.
• Perform outreach STI screening/testing activities.
• Educate providers and patients about STIs and treatment.
• Collect reports during site visits and ensure reporting quality.
• Complete monthly reports and assist in outbreak-related activities.
