Job summary

Northwestern Medicine in Chicago is seeking a Clinical Pharmacist for the Infectious Disease Clinic in the ambulatory setting. Primary scope includes HIV care, including long-acting injectables, OPAT support, and ID-related patient care and medication access.

The role collaborates with physicians, nurses, and other care team members to optimize therapy, educate patients, maintain EPIC profiles, and participate in quality improvement and provider training initiatives.

Qualifications

• Licensed pharmacist with PharmD or equivalent.
• Experience in ambulatory and infectious disease care is preferred.

Responsibilities

• Works under collaborative practice agreements with physicians to manage therapy for chronic conditions.
• Collaborates with care teams to address treatment needs and improve health outcomes.
• Meets with patients to provide medication support and monitoring, directly or with the team.
• Educates patients and families about medications and adherence.
• Maintains EPIC patient profiles and performs medication reconciliation at visits.
• Participates in quality improvement and medication use review activities.
