Job summary

York Post Acute is seeking an Infection Prevention Nurse to lead infection prevention and safety initiatives in a 80-bed sub-acute and long-term care facility. The role focuses on preventing infections, monitoring trends, and guiding staff training to uphold high standards of resident and staff safety.

The position requires an active RN or LPN license, BLS CPR, and specialized infection prevention training, with CIC preferred.

Qualifications

• Current RN or LPN license.
• Valid BLS Certification (CPR) required.
• Completion of specialized infection prevention and control training is required.
• CIC certification preferred.
• Strong analytical and collaborative skills to identify trends and take corrective actions.

Responsibilities

• Identify, investigate, monitor, and report trends in healthcare-associated infections to support timely intervention and prevention strategies.
• Collaborate with facility leadership, clinical teams, and staff to implement and sustain infection prevention practices.
• Ensure ongoing compliance with all infection prevention and control policies, procedures, and regulatory requirements.
• Develop, implement, and evaluate infection control initiatives to reduce risk and improve resident and staff safety.
• Provide staff with education and training on infection prevention practices.
• Participate in the facility’s QAA committee to support continuous improvement efforts.
• Conduct surveillance activities, audits, and assessments to monitor infection control effectiveness and identify opportunities for improvement.
• Serve as a resource and subject-matter expert on infection prevention guidance.
