Job summary

Frisbie Memorial Hospital in Rochester, NH is seeking an Infection Prevention and Quality Coordinator to lead and support infection prevention, quality, and patient safety initiatives across the hospital and physician practices.

You will coordinate surveillance, educate staff, drive performance improvement, and ensure regulatory compliance with Joint Commission, OSHA, CMS, and CDC standards. This role requires accuracy, independent thinking, and strong data analysis skills.

Position Summary

Position Summary: This role leads and supports the implementation of comprehensive infection prevention, quality, and patient safety programs across the hospital and physician practices. The coordinator is responsible for driving performance improvement initiatives, ensuring regulatory compliance, conducting surveillance, and providing education to staff and leadership. This position requires a high degree of accuracy, independent thinking, and specialized clinical knowledge to manage complex activities related to infection control, data analysis, and quality improvement.

• Leads and implements comprehensive Infection Prevention and Quality programs.
• Ensures hospital compliance with regulatory agencies and standards (e.g., Joint Commission, OSHA, CDC, CMS, State DHHS).
• Conducts systematic surveillance and investigations of healthcare-associated infections (HAIs) and other reportable conditions.
• Develops, implements, and evaluates infection prevention and quality improvement initiatives.

Serves as a key resource and educator for staff, leadership, and medical providers on infection prevention and quality best practices.

• Manages data abstraction, analysis, and reporting for infection prevention, quality, and patient safety metrics.
• Coordinates and participates in relevant committees (e.g., Infection Prevention Committee, Antibiotic Stewardship Committee, Quality and Patient Safety Council).
• Maintain current knowledge of infection control practices, guidelines (CDC, APIC, CMS), and relevant literature.
• Provide guidance and serve as a resource to hospital and physician practice staff regarding infection risks, prevention, and control.
• Conduct regular surveillance rounds across the facility and affiliated outbuildings, documenting and resolving compliance issues.
• Identify healthcare-acquired infections (HAIs) through systematic surveillance, reviewing microbiology culture reports and other relevant data.
• Track patients with multidrug-resistant organisms (MDROs) and epidemiologically important infections.
• Investigate significant clusters of infections and adverse events, developing and implementing corrective action plans.
• Report pertinent information to state health departments (NH-DHHS) and national networks (NHSN, CMS, HCA) as required, acting as a liaison during outages.
• Maintain reports and analyze data regarding medical device-use, surgical procedures, hospital infections, and infection prevention interventions.
• Develop and conduct education programs for new and current employees on infection control and quality standards.
• Coordinate and plan Infection Prevention Committee meetings, including preparing agendas, conducting meetings, maintaining minutes, and presenting HAI analysis and risks.
• Collaborate with Employee Health Services to monitor and reduce employee risks, exposures, and illness.

Provide input on infection control during planning phases of construction and remodeling projects, working with the Facilities department.

• Conduct regular audits of infection prevention measures in hospital and outpatient facilities, promoting and monitoring hand hygiene compliance.
• Inspect the environment of care and review procedures, communicating monitoring data to staff and committees.
• Participate in and work with the Antibiotic Stewardship Committee to review antibiotic use, curb unnecessary use, and reduce microbial resistance.
• Generate and distribute facility antibiograms.
• Assist with the organization, preparation, and implementation of performance improvement initiatives aligned with hospital needs and priorities.
• Perform active surveillance to evaluate compliance with best practices for quality and patient safety.
• Participates in the identification, reporting, and investigation of Serious Preventable Adverse Events.
• Update assigned policies and procedures to maintain compliance with regulatory agencies.
• Educate and engage physician leaders, medical staff, and other providers in the organization’s Infection Prevention and Quality Goals and Plans.
• Maintain hand hygiene database and report trends of concern.
• Supports patient safety initiatives, including sepsis care coordination and investigation of adverse events.
• Support real-time evidence-based care for sepsis patients as Sepsis Coordinator, evaluating bundle fallouts and providing feedback to medical staff, nursing, and leadership.
• Provide monthly and quarterly data showcasing performance by provider and key measures related to sepsis.

What qualifications you will need:

Education & Experience:

At least 3 years of hospital experience preferred

Working knowledge of performance improvement tools, data abstraction, analysis and reporting preferred.

Licenses, Certifications & Training:

Certification in Infection Prevention and Control (CIC) or from the Certification Board of Infection Control and Epidemiology (CBIC) required within the first 3 years of employment (preferred upon hire)..

• Demonstrated computer literacy, including Microsoft Office (Outlook, Word, Excel, Access, PowerPoint).
• Ability to become proficient and independent with facility data collection software.
• Excellent verbal and written communication skills.
• Strong independent thinking and problem-solving abilities.
• High degree of accuracy and factual dependability.
• Working knowledge of Performance Improvement Tools and data abstraction, analysis, and reporting.
