Job summary

Sapphire Health Services is seeking an Infection Prevention / Admissions RN to lead infection prevention efforts and manage safe admissions in a skilled nursing setting. You will coordinate surveillance, staff education, and regulatory compliance while collaborating with nursing leadership, hospitals, and families to ensure smooth transitions of care.

The role requires an active RN license in Oregon, CPR/BLS, and experience in infection prevention and long-term care.

Qualifications

• Current, active RN license in the state.
• CPR/BLS certification as required by the community.
• Infection prevention training or certification preferred.

Responsibilities

• Coordinate the Infection Prevention and Control Program per regulations and policies.
• Oversee infection surveillance, trend monitoring and outbreak response.
• Provide infection prevention education to staff and participate in rounds and audits.
• Support admissions coordination, physician orders, meds and transition of care.
• Communicate resident needs and ensure complete clinical documentation.
